How to Add Klaviyo Form Embed to a Page

Copy the EMBED code from Klaviyo

Go to the page in the admin of Shopify

Click the <> button on the right (that is the HTML for the page)

Then just paste the embed code. Click save

Just note - make sure the form is published in KLAVIYO or it will not show. Once published, it will output on the page
